These Terms and Conditions (“Terms”) apply to all users of our services, including medical consultations, treatment plans, and prescription therapies related to testosterone replacement therapy (TRT) and medical weight loss.
By accessing our website or using our services (in person or virtually), you agree to these Terms. Please read them carefully.
You must be 18 years or older to receive treatment. By becoming a patient, you confirm that you meet this requirement.
All treatment decisions are made by our licensed medical providers based on individual evaluations. We do not guarantee outcomes, and results may vary. TRT/BHRT and weight loss therapies involve risks and are not suitable for everyone.
Patients must provide accurate and complete health information and update us on any changes in their health status or medications.

We partner with third-party compounding pharmacies to fulfill prescriptions.
Activ8 Health is not responsible for any issues that occur between the patient and the pharmacy, including but not limited to:
Once a prescription is submitted, the pharmacy assumes responsibility for processing, packaging, and delivering your medication. Patients must contact the pharmacy directly for any order-related concerns. While we will assist, when possible, we cannot guarantee resolution, replacements, or refunds for pharmacy-related issues.
Your privacy is important to us. We comply with HIPAA regulations to protect your personal health information. For more information, please refer to our Privacy Policy or contact us directly.
We may offer care via secure telehealth platforms. Telehealth may not fully replace an in-person exam, and certain cases may require an in-person evaluation.
Prescriptions are issued only after evaluation by a licensed provider. We do not guarantee medication will be prescribed.
Refills require compliance with appointments and lab work. Requests for early refills may be denied.
We reserve the right to discontinue care for reasons including:
