This Telehealth Informed Consent and Authorization governs your use of telemedicine services provided by ProHealth PLLC, doing business as Activ8 Health.

Withdrawal of consent may limit your ability to receive medical services through Activ8 Health.

Risks and Limitations of Telehealth

Telehealth services are not intended for emergency medical conditions.

Patient Responsibilities

This Telehealth Informed Consent shall be governed by and construed in accordance with the laws of the State of Arizona.
