Activ8 Health provides medically guided wellness services, including but not limited to:
Medical Weight Loss Programs (including medications such as semaglutide and phentermine)
Bioidentical Hormone Replacement Therapy (BHRT)
Testosterone Replacement Therapy (TRT)
Associated clinical consultations, treatment planning, and support
Because Activ8 Health works with licensed medical providers and licensed pharmacies, our refund and cancellation policies must comply with applicable healthcare and pharmaceutical regulations.
All sales are final. Activ8 Health does not accept returns or issue refunds for medications, treatments, memberships, or services once they have been ordered, prescribed, processed, or shipped, except where expressly stated in this Policy or required by law.
Federal and state regulations prohibit the return or resale of prescription medications once they have been dispensed or shipped. This policy protects patient safety and ensures regulatory compliance.
All services provided by Activ8 Health are subject to a medical evaluation by a licensed provider. Eligibility for treatment and any prescription is determined only after this evaluation.
If a licensed medical provider determines that you are not medically eligible for treatment before any medication is prescribed, ordered, or dispensed, Activ8 Health will issue a refund of any applicable program or subscription fees paid for that treatment.
Once medication has been prescribed, ordered, dispensed, or shipped, no refunds will be issued, except as required by law.
Your participation in an Activ8 Health begins once you complete the intake process and payment is successfully processed. Fees may include:
Provider review and medical evaluation
Ongoing clinical oversight
Prescription medications (when medically appropriate)
Program support and care coordination

If a billing cycle has already been processed, cancellation will take effect at the end of the current billing period.
Past billing cycles are not refundable.
Refunds are not available if:
A medication has been ordered, dispensed, or shipped
Clinical services have been rendered
A treatment plan has been reviewed or approved by a provider
The request relates to dissatisfaction with results, personal preference, or failure to follow the prescribed regimen
This applies to weight loss treatments, hormone replacement therapy, testosterone replacement therapy, erectile dysfunction treatment, and all other Activ8 Health offerings.
Refunds are generally not issued; however, replacement medications may be provided in limited circumstances:
If your medication arrives visibly damaged, broken, leaking, or unusable, you must notify Activ8 Health within 48 hours of delivery.
Photographic documentation may be required
Upon verification, a replacement may be issued at no additional cost
Refunds are not guaranteed and replacements are the primary remedy
If you receive the wrong medication, dosage, or quantity due to a dispensing error, contact Activ8 Health immediately.
Verified errors will be corrected through a replacement shipment
Returned medications are not accepted
Refunds or replacements will not be issued if:
An incorrect or incomplete shipping address was provided
The package was lost or stolen after confirmed delivery
The medication was left unattended or improperly stored after delivery
The issue is reported after 48 hours from delivery
Treatment is discontinued for personal or lifestyle reasons
This Policy is intended to comply with all applicable federal and state laws governing medical services and prescription medications. Where required by law, Activ8 Health will provide the legally mandated remedy.
